Salt Flats Dentistry has three offices across the south end of the Salt Lake Valley, and all three are accepting new patients.
The Fashion Place office at 6095 S Fashion Blvd sits just off I 15 near Fashion Place Mall and serves Murray, Midvale, Holladay, Cottonwood Heights, Taylorsville, and Millcreek.
The Herriman office at 5069 W 13400 S sits on the Herriman and Riverton border and serves Herriman, Rosecrest, and Bluffdale. Dr. Alejandro Martinez practices here and treats patients in both Spanish and English. This office provides adult dental care and does not see children. The office is open Monday through Friday from 9:00 AM to 5:00 PM.
The South Bangerter office at 3816 W 13400 S serves Riverton, West Jordan, Bluffdale, and South Jordan. It is the only Salt Flats Dentistry location that accepts Medicaid.
All three offices share the same doctors' standards, the same technology, and the same $99 new patient special covering a comprehensive exam, X-rays, and a basic cleaning.

The South Bangerter office is the only one of the three that accepts Medicaid for adults and children.
